Submitting a DMCA Notice

To ensure a timely and effective response, your notice must comply with the DMCA's requirements and include the following information:

- Identification of the copyrighted work claimed to have been infringed, or, if multiple copyrighted works at a single online site are covered by a single notice, a representative list of such works at that site.
- Identification of the material that is claimed to be infringing or to be the subject of infringing activity and that is to be removed or access to which is to be disabled, and information reasonably sufficient to permit the service provider to locate the material.
- Information reasonably sufficient to permit the service provider to contact you, such as an address, telephone number, and, if available, an email address.
- A statement that you have a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
- A statement that the information in the notification is accurate, and under penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.

Response to DMCA Notices
Upon receipt of a valid DMCA notice, Indiana PDF Forms will act swiftly to remove or disable access to the material claimed to be infringing. We will also make a good-faith attempt to contact the party that posted the material, informing them of the situation. It is our policy to document all notices of alleged infringement on which we act.
Counter-Notice and Restoration of Material
If you have received a notification of infringement regarding material you have posted on Indiana PDF Forms, and believe the removal was a mistake or a misidentification, you may submit a counter-notice. Your counter-notice must include the following:
- Your physical or electronic signature.
- Identification of the material that has been removed or to which access has been disabled, and the location at which the material appeared before it was removed or access was disabled.
- A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification of the material to be removed or disabled.
- Your name, address, and telephone number, and a statement that you consent to the jurisdiction of Federal District Court for the judicial district in which the address is located, or if your address is outside of the United States, for any judicial district in which IndiaPDF Forms may be found, and that you will accept service of process from the person who provided notification of the alleged infringement.
Upon receiving a proper counter-notice, Indiana PDF Forms will forward it to the party who submitted the original DMCA notice. If they do not notify us that they have filed an action seeking a court order to restrain you from engaging in infringing activity related to the material on Indiana PDF Forms within 10 business days, we may restore or cease disabling access to the material that was removed.
